(AP) ? Few people want to be wide awake during their colonoscopy exams, but new research suggests too many are getting extra sedation treatment, costing as much as $1 billion yearly in potentially needless services.

Use of anesthesiologists to monitor sedation during colonoscopies and other digestive imaging tests has more than doubled in recent years, and they're used most often for low-risk patients who typically don't need the extra help, the study authors said.

"These services are not harming patients. They're basically giving them a luxury that is not strictly necessary," said the study's lead author, Dr. Soeren Mattke, a senior Rand Corp. scientist. That matters at a time when policymakers are trying to rein in rising medical costs, the authors said.

Patients usually are briefly sedated for a colonoscopy, and some kinds of sedation require monitoring by specialists. That includes use of propofol, a powerful intravenous drug that can cause deep sedation and was implicated in pop star Michael Jackson's death. Anesthesiologist-monitored sedation, with or without propofol, is recommended for high-risk patients, including those who are old or sick or previously had complications with anesthesia.

While propofol sedation is also sometimes given to low-risk colonoscopy patients, the study authors suggest that's often unnecessary. Drugs usually recommended for these patients typically cause lighter sedation, though most patients don't remember anything about the exams afterward. These drugs can be given by the doctor doing the exam, but the study suggests they're often also being given and monitored by anesthesiologists.

The researchers analyzed insurance claims data on more than 6 million U.S. adults who had the colon exams or imaging scopes of the upper digestive tract between 2003 and 2009. The tests are done to screen for colon cancer, acid reflux and other illnesses.

When the study began, 14 percent of these tests included an anesthesiologist. That climbed to more than 30 percent by 2009. The portion of this extra sedation treatment given to low-risk commercially insured patients remained constant during the study and fell slightly in Medicare fee-for-service patients. But the study authors said far too many low-risk patients are still getting this treatment ? more than three-fourths of the commercially insured patients and two-thirds of the Medicare group.

The extra treatment added an average of about $500 to an insured patient's bill in 2009, and $150 to a Medicare bill. In 2009, about 3 million colonoscopies and other digestive scoping tests were done in low-risk patients but included anesthesia services, amounting to $1 billion in potentially unnecessary costs, the study authors estimated.

The study appears in Wednesday's Journal of the American Medical Association.

While some insurance policies exclude coverage for anesthesiologist monitoring for low-risk patients undergoing these exams, insurers sometimes pay for it, Mattke said.

Because of doctor backlash, Aetna, one of the nation's largest health insurers, has indefinitely delayed a policy it tried to implement in 2008 that would have excluded this coverage for low-risk patients, said Dr. James Cross, Aetna's chief of national medical policy and operations.

Reasons for the frequent use of anesthesiologists during these exams vary. Sometimes low-risk patients undergoing colonoscopies and other digestive scoping exams request propofol because they want to be totally unconscious and have heard that it wears off quickly and doesn't cause grogginess, unlike other sedatives, said Dr. John Vargo, a spokesman for the American Society for Gastrointestinal Endoscopy and a digestive specialist at the Cleveland Clinic.

Propofol requires careful monitoring because it has "a narrow window between providing deep sedation and making people stop breathing," and unlike other sedatives, there's no rescue drug to reverse its effects, said Dr. Norm Cohen, vice president for professional affairs at the American Society of Anesthesiologists.

The rising use of anesthesiologists may partly reflect more obese patients and users of Vicodin and other opiate-based prescription drugs, Cohen said. Both may be missed under coding used in the study, but they should be considered at risk because sleep apnea that often accompanies obesity makes sedation trickier, and users of opiate painkillers often require higher than usual doses of sedation, he said.

Vargo said doctors who do the exams can be trained to use propofol in healthy patients, but a journal editorial said some prefer anesthesiologist assistance because it allows them to focus on the colon exam, and if something goes wrong with the sedation, they may not be held legally accountable.

Surprise: Protons bypass hydrogen bonds but still change molecules

ScienceDaily (Mar. 18, 2012) ? When a proton -- the bare nucleus of a hydrogen atom -- transfers from one molecule to another, or moves within a molecule, the result is a hydrogen bond, in which the proton and another atom like nitrogen or oxygen share electrons. Conventional wisdom has it that proton transfers can only happen using hydrogen bonds as conduits, "proton wires" of hydrogen-bonded networks that can connect and reconnect to alter molecular properties.

Hydrogen bonds are found everywhere in chemistry and biology and are critical in DNA and RNA, where they bond the base pairs that encode genes and map protein structures. Recently a team of researchers using the Advanced Light Source (ALS) at the U.S. Department of Energy's Lawrence Berkeley National Laboratory (Berkeley Lab) discovered to their surprise that in special cases protons can find ways to transfer even when hydrogen bonds are blocked. The team's results appear in Nature Chemistry.

Stacking the odd molecules

A group led by Musahid Ahmed, a senior scientists in Berkeley Lab's Chemical Sciences Division (CSD), has long collaborated with a theoretical research group at the University of Southern California (USC) headed by Anna Krylov. In recent work to understand how bases are bonded in staircase-like molecules like DNA and RNA, Krylov's group made computer models of paired, ring-shaped uracil molecules, and investigated what might happen to these doubled forms (dimers) when they were subjected to ionization -- the removal of one or more electrons with resulting net positive charge.

Uracil is one of the four nucleobases of RNA, whose structure is similar to DNA except that, while both use the bases adenine, cytosine, and guanine, in DNA the fourth base is thymine and in RNA it's uracil. The USC group used a uracil dimer labeled 1,3-dimethyluracil -- "a strange creature that doesn't necessarily exist in nature," says CSD's Amir Golan, who led the Berkeley Lab team at the ALS. The purpose of this strange creature, Golan says, is to block hydrogen bonding of the two identical monomers of the uracil dimer by attaching a methyl group to each, "because methyl groups are poison to hydrogen bonds."

The uracils could still bond in the vertical direction by means of pi bonds, which are perpendicular to the usual plane of bonding among the flat rings of uracil and other nucleobases. "Pi stacking" is important in the configuration of DNA and RNA, in protein folding, and in other chemical structures as well, and pi stacking was what interested the USC researchers. They brought their theoretical calculations to Berkeley Lab for experimental testing at the ALS's Chemical Dynamics beamline 9.0.2.

To examine how the molecules were bonded, Golan and his colleagues first created a gaseous molecular beam of real methylated uracil monomers and dimers, then ionized them with a beam of energetic ultraviolet light from the ALS synchrotron. The resulting species were weighed in a mass spectrometer to see how the uracil had responded to the extra boost of energy.

"Uracils could be joined by hydrogen bonds or by pi bonds, but these uracils had been methylated to block hydrogen bonds. So what we expected to see when we ionized them was that if they were bonded, they would have to be stacked on top of each other," Golan says. Instead of holding together by pi bonds, however, when ionized some uracil dimers had fallen apart into monomers that carried an extra proton.

Where the protons come from

"What we did not expect to see was proton transfer," Golan says. "Surprising as this was, we needed to find where the protons were coming from. The methyl groups consist of a single carbon atom and three hydrogen atoms, but methylated uracil has other hydrogens too. Still, the methyl groups were the natural suspects."

To test this hypothesis, the researchers invited colleagues from Berkeley Lab's Molecular Foundry to join the collaboration. They created methyl groups in which the hydrogen atoms -- which like most hydrogen had single protons as their nuclei -- were replaced by deuterium atoms, "heavy hydrogen" atoms with nuclei consisting of a proton and a neutron of virtually the same mass.

The molecular beam experiment was repeated at the ALS, and once again some of the methylated uracil dimers fell apart into monomers upon ionization. This time, however, the tell-tale monomers were not simply protonated, they were deuterated.

Says Golan, "By looking at the mass of the fragments we could see that instead of uracil plus one" -- the mass of a single proton -- "they were uracil plus two" -- a proton and neutron, or deuteron. "This proved that indeed the transferred protons came from the methyl groups."

The experiment showed that proton transfer in this case followed a very different route from the usual process of hydrogen bonding. Here the transfer involved not just an attraction between molecular arrangements that were slightly positively charged and others that were slightly negatively charged, as in a hydrogen bond. Instead it required significant rearrangements of the two uracil dimer fragments, to allow protons of hydrogen atoms in the methyl group on one monomer to move closer to an oxygen atom in the other. Theoretical calculations of the new pathway were led by USC's Krylov and Ksenia Bravaya.

The moral of the story, says Golan, is that methyl groups do not always kill proton transfer. "Granted, this was a model system -- what we did was ionize the uracil systems in the gas phase instead of in solution, as would be the case in a living organism," he says. "Nevertheless, we showed that proton transfer is possible without hydrogen-bonding networks. Which means there could be unsuspected pathways for proton transfer in RNA and DNA and other biological processes -- especially those that involve pi-stacking -- as well as in environmental chemistry and in purely chemical processes like catalysis."

The next step: a range of new experiments to directly map proton transfer rates and gain structural insight into the transfer mechanism, with the goal of visualizing these unexpected new pathways for proton transfer.

Amazon, Discovery Communications Sign Streaming Pact | The ...

Amazon Prime expanded its list of offerings on Wednesday, signing a licensing pact with Discovery Communications that will allow members to stream shows from Discovery Channel, Animal Planet and other cable networks owned by the company.

Among the programs that will now be available are past seasons of "Whale Wars," "Say Yes to the Dress" and "Dirty Jobs," Amazon said.

Among the other Discovery networks included in the deal are?TLC, Investigation Discovery, and the Science and Military Channel.

Since launching its Prime Instant Video service, Amazon has been busy signing content deals with the likes of Universal, CBS and FOX. The company said it now has more than 17,000 titles available for streaming.?

It is all part of leveraging Amazon's deep financial resources to launch a streaming challenger to Netflix and Hulu.?

Seoul shares dip on cyclicals, narrowly keeps 2,000 point level

SEOUL (Reuters) - South Korea's benchmark KOSPI index fell on Monday, weighed by concerns of an economic slowdown in China and as bullish signs from a strong U.S. jobs data were outweighed as expectations of further easing from the Federal Reserve were pushed back.

Cyclical shares fell across the board, as Samsung Heavy Industries , South Korea's third-largest shipbuilder, slid 3.73 percent while Hyundai Steel dipped 3.77 percent.

The Korea Composite Stock Price Index (KOSPI) <.ks11> declined 0.78 percent to close at 2,002.50 points.

Laura Prudom: 'Once Upon A Time' Recap: Red Riding Hood Meets Her Big Bad Wolf

Note: Do not read on if you have not seen Season 1, Episode 15 of ABC's "Once Upon a Time," entitled "Red Handed."

One of these days, I'm sure I'll start taking the "Once Upon a Time" writers' creativity for granted, but thankfully, the show is still fresh enough that every new twist seems to come out of left field. Though the series has already tackled a number of iconic characters in unique ways, the way Jane Espenson turned Red Riding Hood's tale on its head was perhaps the biggest surprise yet in terms of characterization; a welcome surprise, considering how many shows often telegraph their shocks and talk down to their audience.

I spent most of the episode predicting that Granny would be the murderous wolf plaguing the village, especially after we found out that she had been bitten as a child; but I was thrilled that the show chose to avoid the obvious outcome and add a layer of tragedy to Red's story. And while I'm sad that Ruby won't have a Storybrooke version of Peter (did I mention how much I love the "Peter and the Wolf" allusions?), that loss certainly adds more depth to the character, and explains why Red was so supportive of Snow finding her happily ever after with Charming. It was fabulous to see Snow and Red's first meeting, and all the ways in which Red helped train Snow to track so that she would be able to survive on her own while hiding from The Evil Queen. Snow's hesitation over whether to use Margaret or Mary as her alias was also a nice touch.

While last week's dialogue felt a little heavy-handed, "Red Handed" seemed to flow much better, both in terms of characterization and in the well-timed editing between Storybrooke and Fairytale Land. It was also an excellent showcase for Meghan Ory's talent, since Ruby has been underutilized for most of the season. Her progression was compelling, as we watched her grow out of that rebellious teenage attitude (which I guess she's been stuck in for 28 years) into a more mature and confident woman -- one who is finally capable of wearing plaid and jeans instead of miniskirts and punky makeup. Once again, Emma (and now August, in his own way) is affecting change in ways Regina never could've predicted.

Sadly, as Red was discovering herself in Fairytale Land, David and Mary Margaret found themselves mired even deeper in Regina's curse in Storybrooke: first with David's amnesia calling his honesty into question, and then with the discovery of a heart -- most likely Kathryn's -- in a box covered in Mary Margaret's fingerprints. Sadly, no-one in Storybrooke is aware of Regina's penchant for collecting hearts (and doctoring evidence), so it seems as though Mary Margaret will be on the hook for the unlikely crime, at least, until Emma can uncover the truth and exonerate her mother.

This seems like a relatively svelte review, but there's honestly so little to criticize that more words seem unnecessary. While the mythology only crept forward a little, the hour seemed so strong, with such engaging twists that it's nice to simply sit back and enjoy it on its own merits. It still counted as mostly filler, but episodes like "Red Handed" are proof that consistent characterization and a sharp script can make standalone stories every bit as enjoyable as serialized arcs.

Japan marks anniversary of tsunami

PUBLIC life in Japan will pause on Sunday as the nation marks a year since a huge earthquake and tsunami killed 19,000 people and changed the lives of hundreds of thousands more.

At 2.46pm (4.46pm AEDT) trains will stop, shoppers will stand still and people throughout the archipelago will fall silent to mark the exact moment nature's fury was visited on the nation, when the 9.0-magnitude quake set off a catastrophic chain of events.

A national ceremony of remembrance in Tokyo will see the prime minister and the emperor leading silent prayers for those who lost their lives in the country's worst post-war disaster.

Small rural towns along the coast that were turned to matchwood when the tsunami rolled in, smashing whole neighbourhoods and wiping out communities, were set to hold their own emotional ceremonies.

In Ishinomaki, the families of some of those who died were due to visit graves, where they would be leaving flowers in memory of lost loved ones.

Keishitsu Ito said that Sunday would be an intensely sad day for him.

"My wife was washed away by the tsunami. I will put these flowers on her grave," the 80-year-old told AFP.

"I am sad. I have no one to talk to anymore... or to argue with."

In Fukushima and the nearby city of Koriyama, tens of thousands of people were expected to gather in anti-nuclear protests, calling for the end of atomic power in the wake of the crisis at Fukushima Daiichi.

Among those protesting will be some of the nuclear refugees forced to flee their homes in the shadow of the plant as it began venting toxic radiation over homes and farmland.

The government and plant operator Tokyo Electric Power (TEPCO) announced in December "a state of cold shutdown" for three runaway reactors that went into meltdown when their cooling systems were swamped by the tsunami.

But with radiation having leaked from the crippled plant for months, many parts of a 20-kilometre exclusion zone around it are likely to remain uninhabitable for years - perhaps decades - to come, scientists warn.

Radiation fears across Fukushima and beyond were expected to swell the number of those taking part in the protests.

Setsuko Kuroda, an organiser of a two-day anti-nuclear protest in Koriyama, said 20,000 people were expected on Sunday.

"We demand all children are evacuated from Fukushima now," she told AFP.

"Some experts say one third of children in Fukushima were affected by radiation.

"Leaving the situation like this is like they are committing a murder every day."

The anniversary comes after it was revealed cabinet ministers as high as the prime minister had been warned of the possibility of meltdowns at Fukushima in the hours after the waves struck.

For two months after the disaster the government stuck to its line that there had been no meltdown, before finally acknowledging what independent scientists had been warning since March.

Writing in the Washington Post ahead of the anniversary, Prime Minister Yoshihiko Noda said the events of March 11 were etched on the nation's memory.

"We will not forget the loved ones, friends and colleagues lost in the disaster," he said.

Tips for Taking Portraits of Children at Home | Digital Scrapbooking ...

I love, love, love pictures of my kids. What mom doesn?t, right?

I like to take pictures for most major holidays (birthday Christmas, Valentine?s Day, Easter, St. Patrick?s Day, , and July 4th), in hopes someday I will scrap them, but most of the time they don?t get further than Facebook. Maybe someday, when they are older I will get ?caught up.? I doubt it though.

Being a frugal mom, its not always practical to go to a photo studio or hire a professional photographer, especially when I them several times every year. You never know if they are going to be having a ?good moment? during that short amount of time allotted for your session. And besides, I have a nice DSLR (which might have cost a little more than several photo sessions, lol).

My Set-Up

So I bought an inexpensive backdrop stand from Ebay.com, for under $50 I believe. Alternatively, you can make your own with PVC pipe as explained in this article. Then I bought several pieces of neutral colored fabric from the thrift store. One white, one black, and some beiges. Sheets or curtains can work well. And I got some clamps like these to attach the fabric to the stand.

In the morning when the light is good in our living room, and the children are still happy, I bring in the backdrop and adjust it, add a few props from around the house, and grab my camera. A tripod comes in handy too, especially if you want the whole family in the photo.

Tips

  • I usually put their favorite show on the TV so they are happy and distracted. They can see it from where they need to sit for me to get the shot.
  • Snacks (before they get dressed in their picture-taking clothes) are a good idea.
  • Keep the sessions short, especially for little ones. Do 10-15 minutes at a time. Give the child a chance to play and check your photos, then you can come back and do another round if you didn?t get what you want the first time.
  • Use natural light. Position yourself near a large window, or ours is by a sliding glass door.
  • Turn off the flash if possible.
  • Take pictures from different angles. Lay on the floor, stand up high and shoot down, and take some at their eye level.
  • If they want to hold a favorite toy or bottle, that is OK. Take close ups of their faces and crop out the parts you don?t want.

The Results

I am just an amateur mom-tographer, but some of the photos I have taken at home are my favorite ones, better than the pricey studio pictures. Here is one I took today in my living room with the setup I described, for my daughter?s third birthday:
